You have a right to review, on request, a copy of your medical record and/or receive a copy of your medical record for reasonable copying charges.
You may access your medical record information quickly and electronically with a MyChart account.
You may also sign up to view your child’s or another adult’s medical records through MyChart by visiting the MyChart Forms website and completing the appropriate application form.
Please note, medical records from Medical Center Downtown (Formerly Mercy Iowa City) prior to May 4, 2024 will not yet be available in MyChart. With such a large number of patient records to transfer, this will need to happen over a much longer period of time.
Did you receive care at our downtown campus (Formerly Mercy Iowa City) prior to May 4, 2024? If so, you can access limited medical records from March 2022 to May 3, 2024 in FollowMyHealth. Care received on or after May 4, 2024 is accessible through MyChart.
The preferred method to receive medical record requests is in writing or via MyChart. However, when these options are not available, requests may be sent via e-mail or fax.
